PLEASE NOTE: Beginning in January 2022, all students, faculty and staff who are learning and working on campus at MWCC are required to be fully vaccinated against the COVID-19 virus.
Those seeking registration in a campus-based course and who are able to provide documentation of their vaccination status should continue with this registration request. Within one business day of your submission, you will receive an email from MWCC Lifelong Learning with a student ID number and link to an online form. Students will be directed to the home page of Dynamic Forms to create an account in order to access the online submission form required to submit documentation of vaccination status. On this form you will be asked for the student ID number that appears in the email sent from the college. We ask that you complete the online form within 3 days of receiving the email from the college so we may confirm your registration and email you a class confirmation letter.
Students who are unable to submit verification of full vaccination status for any reason may request a reasonable accommodation. A link to this form will be included in the email sent from the college.
